Manage team permissions, protect your sign-in, and recover access to your account.
Your account and workspace
Sign in at app.formbastion.com to manage your forms and responses. Your profile holds your name, email, password, and security settings. Workspace membership determines which projects and submissions you can access.
Invite your team
Workspace owners and administrators can create invitations in Settings. Copy the private invitation link and share it with the intended teammate. They can accept it with their own account.

Protect your sign-in
Use a unique password and set up an authenticator from Your account. Enrollment provides eight single-use MFA backup codes. Store them securely so you can sign in if your authenticator is unavailable.
Recover your account
Save the account recovery code beginning FB-R shown when you register, or generate a replacement in Your account. Use Recover account on the sign-in screen to set a new password with this code. Recovery ends your existing sessions and leaves authenticator protection enabled.
MFA backup codes beginning FB-M are used for the second sign-in factor. They are separate from your account recovery code, and each code can only be used once.
If you no longer have your recovery code, contact support for help verifying account ownership. Never send your password, authenticator secret, or recovery codes.
Review active access
From Your account, you can change your password and end other sessions. Workspace owners and administrators can review team membership in Settings and revoke API keys from Developers.
